We only collect information you choose to send us through the forms on this site. Please don’t include personal medical information in any website form. We will discuss your health privately at your appointment.
The forms on this site

Enquiry form (Contact page): your name, email address, phone number (optional), date of birth, medical category, whether you are a new applicant or renewing, how you would like us to contact you, and your message.

Ask a question form (Blog page): your first name, email address, your question, and whether we may answer it in a blog post without your name.

Subscribe form (Blog page): your email address and your consent to receive new posts by email.
How we use it
We use this information to reply to you, book and prepare for your appointment and, if you subscribe, send you new blog posts. If we answer your question in a blog post, we never include your name or any detail that could identify you.
Sharing your information
We do not sell or rent your information, and we do not share it with anyone outside our practice except the service providers that run this website and our email, and only as needed to provide those services.
Email updates
You will only receive blog updates if you subscribe. Every email includes a link to unsubscribe, and you can also ask us to remove you at any time.
Where your information is stored
This website is hosted by Wix.com. Information you send through the forms is stored on Wix’s servers, which may be located outside Canada, including in the United States, and may be subject to the laws of those countries. Medical records from your appointments are kept separately from this website and are handled under Ontario’s Personal Health Information Protection Act (PHIPA).
